The total amount of your purchases needs to be $50 BEFORE coupons!
You will need to mail the rebate form along with your original receipt(s) with items and purchase price(s) circled by 5/31/13.
You will also need to enter the UPC codes from products purchased on the form.
You will receive a gift card which you should receive in 6 – 8 weeks.
